چکیده

A 55 year-old woman presented to our cardiology clinic with shortness of breath. She had a history of mitral stenosis and chronic atrial fibrillation. The patient was treated with anticoagulant therapy. She reported that she had consumed 5 mg of warfarin daily for more than two years. On admission to the hospital, the patient’s international normalized ratio (INR) revealed an ineffective anticoagulation level (INR = 1.06). She was referred to the echo laboratory to assess left ventricular systolic functions and the quantification of mitral stenosis. Transthoracic echocardiography revealed pericardial effusion, moderate mitral stenosis, and a large left atrial appendage. The Doppler gradient was assessed using the apical window. The maximal and mean gradients were calculated: 12 mm Hg and 6 mm Hg, respectively. The mitral valve area was measured at 1.5 cm by the planimetric method.

A Semi-Automated Algorithm for Segmentation of the Left Atrial Appendage Landing Zone: Application in Left Atrial Appendage Occlusion Procedures

Background: Mechanical occlusion of the Left atrial appendage (LAA) using a purpose-built device has emerged as an effective prophylactic treatment in patients with atrial fibrillation at risk of stroke and a contraindication for anticoagulation. A crucial step in procedural planning is the choice of the device size. This is currently based on the manual analysis of the “Device Landing Zone” fr...

Left atrial appendage mass: is it always a thrombus?

Myxoma is the most common benign tumor of the heart, but it is very rare for it to originate from the left atrial appendage. Distinguishing between a mass, a thrombus, and a tumor in the body of the left atrium with preoperative transthoracic or transesophageal echocardiography is very difficult, even more so in patients with mitral valve disease and chronic atrial fibrillation. A 50-year-old m...

Unusual Case of a Free-floating Ball Thrombus with Preserved Attachment to the Left Atrial Appendage Causing Recurrent Obstruction of the Left Ventricular Outflow Tract

Few cases of a left atrial thrombus without mitral valve disease have been reported. We present an unusual case in which a patient presented to the emergency department with syncope and acute cerebral ischemia caused by a ball thrombus originating from the left atrium (LA). An emergency bedside echocardiogram showed the LA ball thrombus intermittently obstructing the mitral orifice and, at time...
